Please make sure you have the following when you arrive for your appointment:

  • Completed passport application, not signed.
  • Proof of U.S. Citizenship AND current form of photo ID.
  • A check or money order made out to the US Department of State.
  • A check or money order made out to City of Sterling Heights for the application fee & photo.

Passport application may be handwritten or printed in black ink but can not have any corrections or white-out.

Passport Photo Tips

  • Wear neutral colored clothing with no prints or graphics near your head.
  • Hair must be kept off of your face for the photo.
  • Hats and head coverings may not be worn in the photo, except for religious purposes which require a signed statement.
